Blue Book

What is the "Blue Book"?

Originally a literal blue book, the form below is how SOSA members should report hazards and safety concerns as they become aware of them. 

 

Regular and transparent reporting helps us identify trends or recurring issues, enabling improvements in procedures, equipment, or training. By addressing members' safety concerns, we can minimize the risk of accidents that could lead to injury, loss of life, or damage to expensive gliders and equipment.

 

The blue book reporting system is confidential and non-punitive. All concerns will be taken seriously and should be submitted without fear of retribution. 

How it works

If you experience or observe an accident, incident, near miss, or safety concern, please complete a report using the form below. Form submissions are sent directly to the Safety Officer for analysis.

 

The Safety Officer may contact you and others involved to clarify events and gather additional context. You may choose to include your name in the report or remain anonymous.

 

Emerging trends and timely reports will be shared with members throughout the flying season. At the end of the season, all submissions will be reviewed and categorized as part of the Annual Safety Report, which will include recommendations from the Safety Officer on how we can improve.

Be as detailed as possible and focus on the facts.

Reflect on what you could have done better to help prevent or respond to the incident.
